It covers what Tricare actually pays for, why ketamine infusions fall outside that coverage, and the one path that gets eligible patients ketamine treatment without paying out of pocket.
Ketamine has been FDA approved since 1970, but only as an anesthetic. Its use for depression, post-traumatic stress disorder (PTSD), and anxiety is what medicine calls "off-label": a legal and common practice where a physician prescribes an approved drug for a condition outside its original approval. Roughly one in five prescriptions in the United States is off-label.
Tricare, the health care program for uniformed service members, retirees, and their families, ties mental health coverage to FDA approval for the specific condition being treated. Because IV ketamine is not FDA approved for depression or PTSD, Tricare classifies it as unproven for those conditions and excludes it. This applies to every administration route: infusions, injections, and lozenges.
One exception exists. Spravato (esketamine), a nasal spray derived from ketamine, is FDA approved for treatment-resistant depression. Tricare covers it case by case with prior authorization, meaning your provider must document that standard antidepressants have failed before Tricare agrees to pay.

Self-pay ketamine infusions in the Denver market commonly run several hundred dollars per infusion, and a full course typically involves six infusions. That puts many families in the position of paying thousands of dollars out of pocket for a treatment their insurance will not touch.
For eligible Tricare beneficiaries at Mind Spa, the ketamine cost is zero. The six infusions in the Tricare Preferred Program carry no charge to the patient. Your covered TMS course proceeds through your Tricare benefits like any other approved outpatient treatment, subject to your plan's normal cost shares.
